I'm here in SEA. I used Google to look up "Hertfordshire Police" found
the regional office, emailed the info desk that I needed a police check.
They sent an email to the relevant person who emailed me all the forms.
(Hertfordshire was the last county that I lived in whilst in the UK).

I filled in the forms with a photocopy of my passport (that was enough)
and also a cheque in pounds sterling and posted it off.

Post is a bit slow here so still waiting, but there is a government
charter that says they have to reply within 40 days.
