1. You might want to do a different thread on this if you don't get an
answer on this one
2. You should be able to find all the forms you need at
http://www.immi.gov.au/forms_booklets/index.htm. though someone else
might be able to give you a more specific answer
3. Again the above link for the medical forms. You need to go to
specific doctors which you should also be able to find on the DIMIA
web site but you cannot do your medicals until you have a case
officer and case number.
