Not aware of a DIMA form Dave.
We just went to local police station (one tip ensure you keep
receipt !!)
You get the piece of paper back with the 2 blue bands on it , the data
access form, thats what you need to send.
In our case this week, CO wanted a progress report, and requested proof
of application of police check as they are indeed taking 40 days
So we had to scan on the computer and e mail over !.
