Hi Andy,
Can I suggest that you include as much evidence as you can get your
hands on, that shows you and your partner have been living together
for the last four years (DIMA's requirement is to be 'living together'
for at least the 12 months immediately before applying), and are in a
genuine, continuing and exclusive marriage-like relationship. This may
also include things like bills sent to both of you at the same
address, evidence of sharing of household expenses, joint debts, joint
ownership of real estate or major assets. The more evidence, the
better. Make sure you only send certified copies of the originals. And
yes- your statements can be typed and signed, and do not have to be on
a stat dec form.
