Hi,
I am trying to find out how to register with WACOT. All I can find on
the WACOT website is the following form:

Is this what I need to fill in and post?
I have read on several posts that a police check is required. I am
based in the UK. Is this a CRB or is a different type of police check
required, and if so, how do I go about getting such a police clearance
document?
Any advice will be greatly appreciated. I have emailed WACOT twice,
with no reply so far.
