Yes, once you submit the form, it gives you an option to download the
complete form in a pdf format. Save that. Sending it along with the
documents may not be a requirement but doesn't harm either. It has all
the info they need to know who these documents belong to. Also, you may
want to add a small cover letter/index page briefly mentioning your
application ref#, name, MODL you've applied for and then index the
documents attached. Again, not a requirement, just an icing on the cake
:)
