According to:
http://www.cic.gc.ca/english/newcomer/res-oblig.html
the spouse of a Canadian citizen can keep PR indefinitely while accompanying
that Canadian citizen outside Canada. It remains to be seen whether CIC
will try to interpret this more narrowly over time.

PR Card can only be applied for in Canada. The other options are:
- a (single-use) Travel Document; or
- drive over the land frontier; or
- depending on the airline, if she has a US or other visa-waiver passport,
return to Canada using passport only and present to Immigration as a
returning PR
The PR Card is not necessary to be a Canadian PR. Same goes in reverse -
you can lose PR despite having a Canadian PR Card.
Bottom line appears to be bring plenty of evidence of status (eg proof of
spouse's Canadian citizenship plus cohabitation) and always present
yourself to CIC as a returning PR, not as a visitor.
