You need to get "Driver Abstract", i.e. your driving record.

First check RIV at http://www.riv.ca/
Most importantly there is a list of cars can be imported to
Canada, and what potential modification need to be done to that
car.
If your specific model and year is not in the list, most likely
you cannot import the car to Canada.

Yes, get letter of your insurance and claim history, if possible
to 6 - 7 years back.

AFAIK unless you move to a city / town in Canada significantly
smaller that the one you live in USA, then you definitely will
pay higher car insurance rate. A place in Canada will have
higher car insurance rate than comparable place in USA.
