Thanks Jim for your Answers.

I did follow the guide but still was confused as didn't find complete
answers to some of the questions.

I have two more questions:

1) For Identity and Civil Status documents, I am planning to send
Passport phocopies of my wife and her birth certificate. But those
birth certificate from our home country wont list her name. it will
just show her parents name and date of her birth. Will this be
sufficient for proof of Identity and Civil Status documents?

2) Do any photocopies we provide like passport pages, marriage
certificate require notarized or just photocopy is fine?
