Thanks. We got more information from the US consulate and I am posting it here for the
benefit
of others who may face same situation in future

(a) Both parents and the child must appear in person. No exception.

(b) No appointment needed. A walk-in will do.

(c) Payment can be made in cash or credit cards. Takes $82 US

(d) The passport can be picked up after few weeks or can be delivered by registered mail.

(e) It is not necessary that the PP has to be renewed before it expires. It can be renewed
even
after expiry.

(f) It seems condition (a) is enforced to counter international child-abduction cases. We
were
also told that should the consulate find circumstances bit suspicious and the child big
enough
to answer their questions, they may ask the child some questions in private to quell their
suspicion.
I find it funny.
