The airline will take your I-94 and forward it to USCIS (assuming you
fly. If you drive, it's different). In most cases, that's all you need
to do to leave the USA. If the airline by mistake does not collect the
I-94, and you don't notice it until you already left, you should contact
a US consulate for instructions. I believe the current policy is for you
to mail it to an address in Kentucky.
The second thing you may need to do is report your departure. This
applies if you are subject to special registration. At certain airports,
everybody is required to report the departure.
