As you now have a B2 visa, when arriving at the POE, you will have
to fill out the white I94 which is different, not the green I94w.
This page here http://www.cbp.gov/xp/cgov/travel/id_visa/i-
94_instructions/filling_out_i94.xml does not mention any
questions regarding criminal records, and I don't recall answering
any questions like that on one.
The pictures are too small to see, but it doesn't look like they
are on there.
