1- When you apply for the green card at a us embassy you are applying
for immigrant visa. If you apply for the GC in the us it is an not a
visa but an adjustment of status.
2- It says City where you applied for visa or adjustment, if you
applied in the us just write down where. If you applied for the green
card at an embassy say so. It is about where you applied for the green
card.
