Oh dear. You could have a problem.

When you returned after having "lost" the I-94, you were not actually
legally admitted - because you had no right to be admitted, being
subject to a 3 year ban (or is it 10 years?), due to your overstay.

Someone who has not been legally admitted cannot adjust status. There
was a case here of a British woman who was in a very similar
situation. I believe she ended up leaving the US (rather than be
deported), and if memory serves me right was last heard of trying to
get a waiver from the UK.

Advice from an experienced immigration attorney might be a good idea...
