You fill it out as best you can. You give exact dates if available,
approximate dates if that's all you know, and month/year if that is
*honestly* the best you can do. This information is required to see if
you meet the physical presence and continuous residency
requirements... if you comfortably exceed those requirements, don't
worry about it too much.
