If the question asks for "have you ever...", ever means exactly what it says.
If they want information for a certain number of years only, they say so.

Depends. They run background checks, and if your name has some similarity to a
name that is on their lists, they make sure that it is not you on the lists, so
that can introduce delays. They are only supposed to schedule the interview
appointment once the background check is cleared.
Even in the best of cases, this takes 2-3 months. And with the huge backlog that
they have, who knows...
