ipv4 only works that way if it's been NATed at the consumer end. But
it's very common as the ipv4 shortage means that not many ISPs hand out
multiple IP address to end users.

It could, but the "NAT is evil" brigade say there are so many advantages
to end-to-end [which means every host has a unique public IP address]
that they hope it doesn't happen.
