Either.
I tended to use Lao money for small amounts, up to $1 or so. For large
amounts it is simply too bulky to carry around. You would need a bag to
carry $100 worth, its too many notes to fit in a pocket (unless they have
recently introduced larger demonination notes).
For medium amounts I used Baht, for large amounts (air tickets etc)
sometimes US$.
Generally I preferred Baht, but that was probably because I do not usually
carry $.
Change seemed to come in whatever currency was convenient.
More expensive places priced in $, but this was nearly a year ago, perhaps
with the falling $ they have more more to a baht base.
You can use Baht or $ easily in any tourist area, I found the on-the-street
exchange rates in shops etc were OK.
In non-tourist areas I used mostly Lao money.
