If you Google the term JTB you'll find Japan's largest inbound tour
operator, Japan Travel Bureau. Look for their "Sunrise Tours" which are
in English. You can get a lot of good information on their escorted
tours. If you prefer to do it yourself, they honed down their
itineraries that you can duplicate on your own.
With your limited time, you could rush and do as they do:
Tokyo
Tokyo - Nikko - Tokyo
Tokyo - Kamakura - Tokyo
Tokyo - Mt. Fuji - Hakone
Hakone - Nagoya - Toba
Toba - Ise - Kyoto
Kyoto - Nara - Kyoto
Kyoto -
Kyoto - Tokyo
Tokyo (Depart Narita)
Or add another day in Tokyo at the top and depart from Kansai
International, Osaka.
Frankly, the above is very rushed for me but quite doable.
