In addition to the CDC site listed earlier (always the first stop
on these things), I would also suggest at least considering checking
with a doc that specializes in travel medicine, especially to some
of the more exotic places or if you plan to go off the beaten
tourista path.
You can find someone local through the International Society of
Travel Medicine at http://www.istm.org/. They really don't seem to
have a lot of consumer-type things on the site.
