The insurers are onto that "cancel and rebook" idea. For exsmple, this
is CSA's pre-existing ondition waiver conditions. Note condition #3:
"The Pre-Existing Condition Exclusion is waived provided you meet all
of the following requirements:
1. the payment for this plan is received prior to/or within 24
hours of your final payment for your Covered Trip; and
2. you are not disabled from travel at the time you make your plan
payment; and
3. the booking for the Covered Trip must be the first and only
booking for this travel period and destination."
If you haven't made the final payment on the trip you can get coverage
for pre-existing conditions with most CSA plans (all except Freestlye)
by buying the plan no later than 24 hours after making that final
payment.
If you're part that 24 hour window and if the pre-existing medical
condition that may cause the cancellation belongs to a non-traveling
family member, just buy a policy that does not include non-traveling
family members in their definition of the pe-existing condition
exclusion. CSA does not. Neither does Global Alert and there's others.

during the 180-day period immediately prior to your effective date for
which you or your Traveling Companion or Family Member IS SCHEDULED OR
BOOKED TO TRAVEL WITH YOU . . . ":
