Just wondering. On what do you base the above opinion. When they do an
investigation they create a team that includes experts who are not
government employees. There are good reasons for having a thorough
investigation. For example if it was human error, they would
investigate reasons for the human error. It could be poor training. It
could be fatigue. Many transportation employees are limited in their
hours of service because it has been determined that fatigue is a major
cause of human errors. It is important not only to determine the cause
of an incident but to recomend safety improvements to prevent a future
incident.
