I've made quite a few trips to Germany (from the US) and have always
rented a car and had very little trouble getting around. Driving is
not that difficult or really that different from parts of the States,
IMO, and one has a lot more freedom on when and where to go. I've
always been able to find parking in the cities, although learning how
to ask someone in German where parking might be is a big help. (Don't
count on hotel desk personnel speaking English well, no matter what
you hear.) My acquaintances that use trains seem to have more of a
hassle and it takes them longer to get around. Handling baggage is
more of a pain with trains, too.

You do need to bone up on signage in German. I've mentioned before
that on my very first visit I was driving from the Frankfurt airport
rental parking garage and was immediately faced with a sign that said
"Ausfahrt", pointing one way, and "Ausgang", pointing another.
