Ah yes, the Russian visa form. I got to fill out 4 of them, for 2
underage nephews and a partner, and myself. Easier than tax forms, but
less familiar.

For line 17, I put my Blue Cross #XXXXXX number. SF consulate said
they needed to have something in that line. I filled in genuine if
possibly irrelevent info. It worked.

Line 10 just needs to be the starting city. If you know them all, go
for it.

IMPORTANT: The visa costs the same for up to 30 days. The visa validity
will be for your requested entrance and exit dates. It is prudent to
give yourself some slack for departure--even just round up to 30 days.
It is easy to be delayed by a few days and there is no reason not to go
for the full visa validity.

I did not enter 38, work address in Russia, since this was tourism. I
did not enter 39, hotel address, because it was not yet determined. I
claimed that we all did our own paperwork, #40, because they mean did
you pay someone for this. I was going to let teen-age boys decide how
to enter the questions about experience with explosives?

Some of the weirder questions were copied from the USA long form (males
of military age from suspect countries). The Russians dropped the
question about prostitution.

Good luck. Our trip was not entirely fun, but very interesting and
with some great experiences.
