Wouldn't taking a quick side trip to another country - say Switzerland, not
that far by train from Milan - for a day or two and then re-entering
"re-set" the clock?

I can't comment on the penalties specific to Italy but guess they could
include arrest, deportation, a ban from re-entering the state and possible
fine or imprisonment. That said, as DFM has stated, these penalties will
only apply if you (a) get caught and (b) they want to make something of it,
both of which sound rather unlikely.

Like all visa/overstay questions, it comes down to this - do you want to
break the law and take your chances, or conform with the law and take no
chances?

IANAL
