You should probably add Yellow Fever to the list if you're going to the
Amazon. Some authorities require a YF certificate before they'll let you
enter/leave. You'll also want to look at an anti-malarial regime, which may
need starting before you enter a malarial zone depending on what you choose.

As the previous poster recommended, you should really talk to a travel
clinic before you depart. Leaving it to when you get to Lima may mean you
spend time suffering from the possible side effects of the vaccinations.
