I've drunk many iced drinks, eaten many unpeeled fruits in Mexico
without problems. The fruits were carefully washed with hotels'
allegedly disinfered water, however. With much care, I still got the
bug once eating tacos in a restaurant in Mexico.

Local people may get exposed to certain local bacteria and build up
resistance whereas tourists may not have it. I was in a tour visiting
Penang, Malaysia with 20 people. We ate Indian food with fried chicken
and patries and all got severe food poisoning for 3 days, except a
couple from Portugal, who ate the same meal but were fine. They must
have been previously exposed to that bacteria and developed
resistance! That were 3 nights of delirium, hitting the toilet 3,4
times a night, ruining the vacation! I reported the incident to the
local police to make sure they monitor that place!

Street vendors in third world countries may not wash their knives,
chopping blocks, serving utensils etc... regularly, therefore
infecting customers.

Boiling hot fresh food, served in disposable or known-clean plates and
utensils should be OK.

Immodium has worked wonder for me overseas.
