True. However, "New York City" is standard informal usage.

True. The nearest miss for the New York pattern is Oklahoma, where
Oklahoma City is in Oklahoma County; but in this case the word City
really is part of the city name, so it doesn't count. This latter
pattern of names is also repeated in Arkansas, Idaho, and Iowa, but
in each of these cases the city and county are in two different
places in the state (that is, Idaho City, ID, is not in Idaho County).

Not really; people rarely have occasion to talk about counties.

Each borough *is* also a county, in most cases having a different name.

Yes, and it's within New York City. The five boroughs are:

Manhattan = New York County
Brooklyn = Kings County
The Bronx = Bronx County
Queens = Queens County
Staten Island = Richmond County
